Information Collection and Use
NYSNA gathers personal information during the registration process. The following information is
necessary to obtain a certificate of completion: name, address, profession, and e-mail address.
The New York State Education Department (NYSED) requires the following information for participants
of the New York State mandated Identification and Reporting of Child Abuse and Infection Control and
Barrier Precautions courses: name, address, social security number, date of birth, license number
and license state (if licensed), and profession.
NYSNA is the sole owner of the information collected on this Web site. Personal information is
disclosed only in the following instances:
- When an accrediting agency requires verification that an individual has taken a course;
- When an agency requires us to report all persons completing a course (Florida CE Broker, NYSED).
Credit card data is secure and is not retained after the payment transaction has been completed.
We keep no record of a customer's financial information.
Log Files
The New York State Nurses Association uses log files to track the number of people coming to our Web
site in order to analyze trends and demographics. The logs are not linked to personally identifiable
information.
Cookies
Cookies are text files created by a Web site that store information on your computer, such as
your preferences when visiting that site. We use cookies to grade your exam. No private
information is recorded in our cookies; the information contained in our cookies is not
shared with any other organization.
